2008 New Jersey Ride Against AIDS
In order to participate for The NJ Ride Against Aids, you must download, print, complete, and return the following forms.
- Rider/Crew Registration Form
- Pledge Form
- Other Forms to be signed
- Includes:
- Check list
- Code of conduct and safety
- Confidential emergency medical information
- Waiver
- Photo release
- Vehicle information (for crew with vehicles only)
